Recent changes by the Health and Safety Executive (HSE) to the RIDDOR report forms required some updates of the Helpline module question set. These changes impact the accident and near miss/dangerous occurrence case types.
All the changes and enhancements addressed as part of these changes target helpline Module only. The changes are listed below:
Feature 1: Changes to ‘Accident’ Case type
'Incident details' section
- New question added to record the Title of person, This appears as 'Title'.
- Response to gender question changed to include 'Prefer not to answer.
- The age range capture is changed from preset selection to free text.
- Change to 'Address' field label. This now reads 'Address and Postcode.
- Wording changes to few responses in dropdown for 'type of disease' question.
- Response to 'type of injury' question changed to include ‘Crush', ‘Scalping’ and ‘Internal injuries’ in answer dropdown for 'type of injury’ question.
- New question added to record the environmental conditions at the time of accident.
- Wording changes to few responses in dropdown for the 'Best description of accident/incident' question.
- New question to record any other relevant information enabling response in free text. This appears as 'Is there any other relevant information which describes what happened?'
'Details of any equipment/machinery involved' section
- Section name changed from 'Details of any equipment/machinery involved' to 'Details of any equipment/machinery/substances involved'.
- Wording change to the question ‘Was any equipment or machinery involved in the accident/incident?' to include substances.
- Wording change to the question ‘Type of equipment/machinery’ to include substances.
- Wording change to the question ‘Name of equipment/machinery (including any asset or serial numbers)’ to include substances.
Feature 2: Changes to 'Near Miss/Dangerous Occurrence' Case type
'Incident Details' section
- A help text has been added for the question ‘Why did the near miss/dangerous occurrence happen?’ This has a help text that reads 'Only record accurate information based on facts and not assumptions.
- New question added to record 'Type of dangerous occurrence' with range of response to choose from in a dropdown.
